Quilt Studio Intake Form

This is my personal intake form and it works well for my studio.  Feel free to use it if it works for you or just take some ideas and add your own personal needs to it.

Some suggestions could be: 

• Cost per Hour
• Signature
• Completion Date

The pdf is two pages, print page one and then print page two on the back of page one. 

Page one has the intake details and page two is where I write my personal notes (measurements, design ideas, who the quilt recipient is, etc...).

Then cut the page in half and you will have two complete forms ready for use!  Please consider subscribing to the YouTube page, it really does help a lot.
